BRONX, New York—Essen Health Care has been selected by the Centers for Medicare and Medicaid Services (CMS) to participate in the “Guiding an Improved Dementia Experience” (GUIDE) model, a new Medicare alternative payment model. This model is designed to enhance care coordination and provide support to people living with dementia and their caregivers.
By using the GUIDE Model, Essen Health Care will join nearly 400 participants nationwide in developing dementia care programs (DCPs) that focus on improving access to essential services and support, such as respite care. The GUIDE model seeks to empower caregivers and strengthen those with dementia’s ability to remain in their homes and communities.

GUIDE participants represent a range of health care providers, including large academic medical centers, small group practices, community-based organizations, health systems and hospice agencies.

Launched on July 1, the GUIDE model is a new payment system that offers supportive services for dementia patients, including comprehensive, person-centered assessments, care plans and care coordination. Model participants will have 24/7 access to an interdisciplinary care team member or helpline and receive respite services. The model aims to highlight the role of care navigators, who assist dementia patients and their caregivers in accessing clinical and non-clinical services, such as meal delivery and transportation, through community-based organizations.
