Partnership will expand access to high-quality in-home care for patients in Southeast Texas with multiple chronic conditions

NASHVILLE, Tennessee & HOUSTON—Monogram Health, a value-based multispecialty provider of in-home care for patients with multiple chronic conditions, and Memorial Hermann Health System, a nonprofit health system committed to creating healthier Houston communities, launched services to provide care for commercial and Medicare Advantage (MA) members of Memorial Hermann Health Plan, the insurance division of the Houston-based health system.

This joint venture between Monogram and Memorial Hermann brings together Monogram's expertise in providing multispecialty in-home care with Memorial Hermann's care network that includes its physicians and more than 270 care delivery sites across Greater Houston. The launch of services for Memorial Hermann Health Plan members aims to deliver holistic, patient-centered care that addresses the complex needs of individuals living with chronic kidney disease and its causes, including diabetes, high blood pressure and metabolic disorders.

This initial population launch sets the stage for expanding the program to additional populations and health plans, further advancing the joint venture's goal to improve care for patients with multiple chronic conditions.
