NEW YORK, N.Y. (October 25, 2016)—Alliance Homecare—a concierge home health care provider serving individuals and families in New York City, Long Island, Westchester and the Lower Hudson Valley, announced the kick-off of its 10th anniversary campaign, Seniors @ Home, with three new initiatives that reinforce the company’s white glove, high-touch approach. As part of the anniversary campaign, Alliance Homecare will launch the Rehab To Home Transition Program; personalized CEO Concierge visits for their clients; and a Healthy Aging speaker series to educate the public on aging-in-place and the importance of planning for the future.
Alliance Homecare’s Rehab To Home (RTH) Transition Program provides extensive, multi-level support enabling patients to return home with appropriate care after a sudden life-changing event with complicated medical needs. The RTH Transition Program encompasses expert counsel, coordination, training and continuous in-home services from a multi-disciplinary care team that give families a viable option over a long term stay in a nursing home.
The RTH Transition Program is a partnership with Frangeli Consulting & Design, a highly specialized professional design firm that oversees home modification designs specifically for the medically and mobility compromised. Following a comprehensive home safety analysis and assessment, Frangeli designs the necessary modifications, including recommending new adaptive equipment, to ensure safe home access, showering, sleeping and self-care.
“Alliance Homecare and Frangeli have a unique understanding of our client’s medical and residential modification needs,” said Greg Solometo, co-founder and CEO of Alliance Homecare. “It is our goal through the Rehab To Home Transition Program to facilitate maximum independence in the least constrictive environment, decreasing the risk of readmission to a hospital system. We have the experience, skill set and capabilities to ensure that every person’s home remains a place where he or she can live comfortably, safely and independently.”
Solometo added, “Social work and discharge planners often don’t have other options for a family in these circumstances and are limited to suggest a skilled nursing home as the only option. To a patient population in their early or middle adulthood (or really at any age), this limitation is not acceptable. We have built this program to give families a more favorable option than the market has previously provided.”
The RTH program is currently limited to privately funded scenarios or through catastrophic insurance, workers compensation or disability. For those seeking a more financially accessible option, Alliance Homecare offers Rehab To Home Lite, a scaled-down version of the traditional transition program, which provides solutions at a lower cost price point. Over time, Alliance Homecare hopes to expand the RTH program further through government partnerships to cover a larger number of people to be cared for at home.
Alliance Homecare is launching the CEO Concierge visit, which is representative of the company’s overarching commitment to providing comprehensive quality, holistic and collaborative care. Alliance Homecare’s Director of Relationship Management, Sheila Kolt, accompanies Solometo on each home visit to review the current plan of care and to hear feedback on the caregivers and care management team. They also highlight some of the additional services a client or their family may be interested in such as (but not limited to) specialty companionship to visit the Met or opera, a facilitator to read the Sunday paper, religious affiliate visitation, spa services in the home or bill paying assistance.
“Every client’s needs are different and it’s our responsibility to bring valued services we have grown to trust to all of our clients. At least they know what options are out there and feel like they are not so isolated in the home,” said Solometo. Following the visit, a survey is sent to the adult children/POA to secure additional input. They are then invited to meet with Solometo and Kolt, again, to discuss both surveys and the recommended plan of action.
Alliance Homecare is also announcing a speaker series program, “Experts Speak: Healthy Aging—Planning Ahead” (presented by Alliance Homecare), which will commence in early 2017. In addition to the Alliance team, the informative panel presentations will feature an attorney specializing in elder care and trusts and estates, a daily money manager, and an estate dissolution specialist. The series will be geared towards boomers who are facing challenges with a loved one.
