The companies plan to support the expansion of remote patient monitoring by introducing home-based hospital care at Lee Health

NASHVILLE, Tennessee—Biofourmis, a technology-enabled care delivery company, has partnered with Lee Health, a not-for-profit health system, to launch a new hospital at home program. The home-based hospital care model aims to expand the companies’ remote patient monitoring (RPM) programs and support patients across the care continuum.

As part of the agreement, Biofourmis intends to support Lee Health’s RPM service line. The companies’ hospital at home program, which will be the first of its kind in Southwest Florida, aims to provide an alternative inpatient site of care whereby patients can receive hospital level care in the comfort of their own home. Hospital at home programs aim to bring the resources, supplies and care teams to patient homes, which can improve patient satisfaction and clinical outcomes.

“Improving patient experience and supporting an environment that best promotes a patient’s health and well-being is of utmost importance for Lee Health,” said Jennifer Higgins, chief nursing officer at Lee Health. “The hospital at home model utilizes technology at a new level, giving us the chance to allow patients to heal in their own home with support from our compassionate staff of nurses and doctors.”

Biofourmis offers a platform aiming to provide continuous monitoring of patients, including integrated personal emergency response system, arrhythmia detection, fall detection and continuous vitals monitoring, that could integrate through a single pane of glass to their 24/7 command center nursing team.

In an effort to enhance the hospital at home experience, Biofourmis plans to integrate with Lee Health’s electronic health record system to ensure patient data is accessible for the continuity of care and the management of acute and post-acute care. Additionally, the program intends to coordinate and track other health services that are simultaneously occurring.
