Home-based health care provider awarded special distinction for exemplary care model

BRENTWOOD, Tennesse—Compassus, a national provider of home-based health care services, recently received the first Age-Friendly Care at Home certification for hospice in the country, awarded by Community Health Accreditation Partners. CHAP is a nationally approved accrediting organization that assesses the quality of health care providers. By achieving this certification, Compassus has demonstrated the integration and application of key principles and practices in providing high-quality hospice care for older adults.

CHAP developed the Age-Friendly Care at Home certification as an extension of the Age-Friendly Health Systems program, an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement in partnership with the American Hospital Association and Catholic Health Association of the United States. To be certified, a provider must support a care model that follows four evidence-based elements of high-quality care, known as the “4Ms”—what matters most, medication, mentation and mobility.

“Achieving Age-Friendly Care at Home certification reaffirms the work Compassus does every day to provide hospice care that focuses on a person’s all-encompassing needs,” said Jen Hale, Compassus vice president of clinical quality and standards. “We are encouraged by the expansion of these standards to home-based care and honored to be distinguished as an Age-Friendly Care at Home provider at a time when more people are choosing to receive care in their home environments.”

The Compassus care delivery model is nationally recognized for its person-centered, outcomes-focused approach that follows evidence-based best practices, including those outlined by the Age-Friendly Care at Home program, to provide measurable impact on serious illness at the end of life.

“With most people desiring to receive care at home as they age, achieving certification and extending the principles of Age-Friendly Care to the home setting is significant,” said Khai Nguyen, national medical director for CHAP. “We congratulate Compassus on being the first in the nation to achieve this designation and for their continued commitment to providing age-appropriate care at the end of life.”
