BRONX, New York—Essen Health Care, a New York-based medical services provider, announced it has been selected by the Centers for Medicare and Medicaid Services (CMS) to participate in the Guiding an Improved Dementia Experience (GUIDE) model, a new Medicare alternative payment model. The GUIDE model aims to enhance care coordination and provide support to people living with dementia and their caregivers.

Under the GUIDE model, Essen Health Care will join nearly 400 nationwide participants to help develop dementia care programs and improve support and services access. The company said it is dedicated to enhancing care for vulnerable populations through innovative approaches.

The GUIDE model is a payment system that intends to provide supportive services for dementia patients, including comprehensive, person-centered assessments, care plans and care coordination. Companies that participate in the GUIDE model include large academic medical centers, small group practices, community-based organizations, health systems and hospice agencies.
