TYLER, Texas—Family Tree Private Care, a private care operator in the U.S., announced its merger with and into Choice Health at Home, a home health, hospice, personal care and rehabilitation services operator. The merger aims to establish Choice Health at Home as a nationwide operator that offers a full continuum of in-home care to patients.

Choice Health at Home seeks to provide care throughout the Southwestern U.S. With the addition of Family Tree’s private care operations in Texas and Colorado, the company said it intends to strengthen its presence in those states. Additionally, the transaction aims to provide the infrastructure for a long-term strategic plan that serves as a single destination for in-home care in the region.
