BURKBURNETT, Texas—HomeWell Care Services, an in-home care services provider, announced the relaunch of its “GoHomeWell” post-medical care program. The revamped program, which was initially launched in 2019, aims to provide franchise owners with advanced tools, resources and data-tracking capabilities to deliver outcomes-driven care and strengthen partnerships with health care providers. The program relaunch intends to focus on reduced hospital readmissions for users.

The company said GoHomeWell has helped support its personalized homecare services since its inception. The program aims to assist clients during their recovery journey at home following an extended hospital stay, rehabilitation stay or surgical procedure. The relaunch seeks to offer several new enhancements, including a readmission rate tracker. Cone said the tracker tool aims to empower care teams to measure, monitor and improve 30-day readmission rates, and offers data-focused client care.
“The readmission rate tracker alone highlights our dedication to both data and personal care—something that few competitors can offer,” Cone said.
In addition to the GoHomeWell program relaunch, the company introduced a newly redesigned marketing hub, which is a storefront and platform that aims to equip franchisees with updated collateral, operational resources and marketing tools.
