PHOENIX—MD Home Health, an Arizona-based home health agency, announced the expansion of its health care services to include in-clinic care, virtual care, remote patient monitoring, house calls and hospice care. This expansion has made the company one of the first privately-held home health agencies in Arizona to offer an omnichannel health care approach. The company said this expansion allows for increased access to comprehensive, convenient health care for residents across the Phoenix, Arizona, metro area.

MD Home Health’s new offerings include:

  • Virtual Visits: Allowing patients to connect with their health care providers via online videos for convenient care and consultations.
  • House Calls: Expected to be offered by the end of the year, house calls offer convenient medical care treatment by a nurse practitioner or physician in the home when a trip to a clinic is inconvenient, too difficult or not possible.
  • Remote Patient Monitoring (RPM): Enables patients to share key health metrics via a variety of connected devices, such as heart monitors, glucometers, pulse oximeters, thermometers, body weight scales and blood pressure cuffs. These metrics allow the sharing of vital data with a provider in real-time. Additionally, RPM has been shown to reduce hospital readmissions, improve patient satisfaction and improve health outcomes. 
  • Hospice Care: Offers care and support for patients, their families and loved ones. Hospice care meets the patient where they are, such as their home or an assisted living facility. New hospice patients are expected to be admitted this fall.
