The dashboard aims to give a full view of patient population across the care continuum & enhance continuity of care

OVERLAND PARK, Kansas—WellSky, a global health and community care technology and services company, announced the launch of a new patient panel, which will be available in the WellSky “CarePort” Connect solution. The panel aims to equip providers with critical and holistic knowledge of their patient population, as well as expand provider visibility to help ensure patients receive care at the right time and in the right setting.

The patient panel intends to offer a customizable, 360° real-time view of an organization’s patients by care setting and highlight cohorts tied to clinical and operational goals in order to help manage and prioritize care. With an at-a-glance view into patient locations, providers using the panel will be able to identify patients who are currently in the hospital, as well as anticipate discharges from both acute and post-acute care.

The company said that providers will be able to track patients’ rising risks and anticipate patient needs with the panel’s predictive care planning tools. By tailoring views to clinical roles and focusing on team-based patient management, the panel aims to help providers filter for assigned patients or specific patient attributions, making it easier to monitor and intervene with patients. This approach seeks to help remove guesswork from patient engagement and support efforts in meeting program goals and delivering quality care.

The patient panel intends to help providers highlight recommended workflows, outstanding tasks and care team discussions, which can equip providers with actionable insights to inform decisions and improve coordination.
