Continuing Care at Home (CCaH) is a growing care model in the US that allows older adults to age in place with the benefit of coordinated services and oversight of a professional care manager. CCaH programs can cover certain daily benefits for private duty home care, as needed, and even assisted living and skilled nursing if the member simply cannot remain in their home.
